Procedure For Admission

To apply for admission to the Unitarian Universalist House, a completed application and medical history form must be submitted with a non-refundable application fee of $50.00. The medical staff reserves the right to ask for additional information, examinations or tests. All such medical expenses incurred will be the responsibility of the applicant. Each resident in the House must be under the care of a physician. You can elect to remain under the care of your present physician or select one from the list of physicians associated with the Unitarian Universalist House. Our physicians are associated with local hospitals which include Roxborough Memorial and Chestnut Hill. Advanced Directives stating medical treatment decisions must be signed appointing someone to make such decisions in the event one is incapacitated. Both documents must be submitted or signed on the date of admission.
